As part of our client-centered model, we are looking for a compassionate and organized Elder Care Coordinator to serve as a key resource and advocate for our clients and their families.
Position Overview:
The Elder Care Coordinator works directly with elderly clients and their families to assess care needs, develop and implement care plans, and provide ongoing support throughout the aging journey. This role bridges the gap between legal planning and real-world care, helping families understand their options and connect with appropriate medical, social, and community resources. The Coordinator works closely with attorneys to ensure care decisions align with legal strategies and long-term goals.
The role requires collaboration with families, healthcare providers, and community resources to support clients' independence, dignity, and overall well-being.
The ECC should be comfortable working with discharge planners in hospitals, ALF/SNF facility administrators, being with families during care-planning meetings at home or in a facility.
Key Responsibilities:
• Develop a “sphere of influence” in the elder community to assist the firm in building out the Life Care practice; foster relationships throughout the community with doctors, ALFs, medical facilities, appropriate associations.
• Conduct in-depth assessments of clients’ medical, social, and emotional needs.
• Assist with intake process, serving as a resource to PNCs on Life Care Planning options; participate in client onboarding process.
• Create and manage personalized care plans that align with legal and financial planning strategies.
• Act as a liaison between families, medical providers, home care agencies, and other service providers.
• Monitor clients’ well-being and adjust care plans as needed in collaboration with attorneys and family members.
• Provide education and support to families on issues like dementia care, facility placement, home care, Medicare/Medicaid, and veterans’ benefits.
• Assist with transitions to higher levels of care and advocate for clients within the healthcare system.
• Maintain clear, detailed documentation to support both care coordination and legal planning.
• Maintain relationships with local ALFs, SNFs, and home care agencies for appropriate client placement.
• Attend community events with local elder/LTC service providers as well as health fairs.
• Stay informed about current elder care practices and available resources to provide clients with up-to-date information and options.
Essential Qualifications:
• Bachelor’s or master’s degree in social work, Nursing, Gerontology, or a related field.
• Minimum 2 years of experience in care management, senior services, or a related area. Experience working with senior citizens and the healthcare/long-term care system is a must!
• Knowledge of long-term care systems, Medicaid planning, and elder care resources.
• Strong understanding of community resources that serve the elderly.
• Strong communication and interpersonal skills, with the ability to support families during emotionally difficult times.
• Organized, resourceful, and comfortable working as part of a multidisciplinary team.
Preferred Qualifications:
• Experience working in or alongside an elder law or estate planning practice.
• Familiarity with Medicaid eligibility and application processes.
• Certification in Care Management (e.g., CCM, CMC) is a plus.

This role typically involves working in various settings, including clients' homes, Assisted Living Facilities, Rehab/Skilled Nursing Facilities, working with hospital discharge planners or other healthcare facilities, and community centers. It requires flexibility to travel within the community and adapt to clients' changing needs.
