Role Description
This is a hybrid role for a Nurse Navigator at Homecare Hub. This individual will perform assessments of patients and help with placement of patients into small homes, and as well oversee a cohort of patients to assure their clinical healthcare and non-clinical needs are met.
This hybrid role is located in La Crosse, Wisconsin and surrounding areas. There may be occasional travel across the state. Most in person work will be local, and there will be a component of work from home as well.
